How to ask for legal assistance and file a complaint in POEA

To file administrative cases and illegal recruitment cases against a violating recruitment agency that deployed you abroad, an OFW has to ask assistance from the Legal Assistance Division (LAD) of the Philippine Overseas Employment Administration (POEA).
Office Location: Legal Assistance Division (LAD), 4th Floor Blas F. Ople Building, Ortigas Avenue corner EDSA, Mandaluyong City of the POEA.
Documentary Requirements: OFW Record, verification form
Fees to be paid: None
Step 1: Fill out a verification form. (This can be obtained from the security guard of the office.)
Step 2: Present the accomplished form at the Licensing branch (Window #3) for verification of status of the agency/staff of the agency.
Step 3: Once the verification form is verified together with the OFW information sheet, present it to the LAD Legal Office.
Step 4: Once you are called, proceed to the legal officer's desk for counseling and legal assistance in the preparation/filling out of a complaint form.
Step 5: Present the accomplished complaint form to the LAD legal officers and swear under oath as to the truthfulness of the facts stated in the complaint.
Step 6: File the complaint at the Conciliation Unit (located at the 2nd floor of the building) if it is necessary to be conciliated or at the Docket and Enforcement Division, 3rd floor.
Filing of an administrative complaint may take at least 50 minutes, and a complaint about illegal recruitment may take 75 minutes of processing.
